Hello. The usual practice (although Gmsh is versatile and used in many different ways) is to define Physical entities in Gmsh, which are then exported in the mesh and read by the solver program (FreeFem++, Escript-Finley, libMesh...), where loads, boundary conditions, etc., can be applied according to the tags on the domain or boundary elements corresponding to the Physical Surfaces and Physical Volumes.
It is possible to link Gmsh and your solver more closely, but that requires a little bit of programming; communication via the mesh output file suffices for most.
