[Gmsh] "Wrong number of adaptions" error

Ah, that's disappointing.

I implemented a simple workaround in my *.msh export code by adding
dummy nodes in my lower-order elements to match the number of nodes in
my higher-order elements.

I might dig in to the Gmsh code one day and see if I can address this issue.
