[Gmsh] Missing tets in Gmsh

Dear Dr. Christophe Geuzaine and Dr. Jean-François Remacle:
I found a problem with Gmsh in which some tets are not included in the
final .msh file.
I believe the problem is in the “writer procedure” that generates .msh and
.vtk files.
The input .geo file is attached and was processed with Gmsh 3.0.5.
Information for the
Build is included in the attached picture. In Gmsh I pressed (3D, Set order
2, Optimize 3D, Save).
The problem that can be found in the output .msh file is the following:
Nodes 19092, 19733, 19378, 19951 are all located on the top surface, and
form two triangles
(19092, 19733, 19378) and (19378, 19951, 19092).
These triangles are included as elements in the file. However, there are no
tets in the file for
which either of these triangles is a face.
I really appreciate any thoughts you may have to help me with this issue.
