[Gmsh] Max element growth rate

Hi Christophe,

I am trying to implement max element growth rate into GMSH, and have some
questions:
1. Are you planning to use p4est in a new field in order to support max
element growth rate.
2. If not 1, what an algorithm will be used for that?
3. Is it already possible to take a look for a new field?
4. Will the new field be added in gmsh 4.5 ?
