I am able to merge the CAD model on top of the mesh, but I don't know how to link the mesh to the CAD model so that the extension to higher order will use the CAD model. Is this possible? If so, how could I do it?

There is an experimental mesh matching feature (gmsh -match): give it a try. (CC: Bastien, who coded this.)
